Abstract: 

Introduction: Coronary artery bypass graft is one of the most common surgeries in hospitals. Almost 50% of patients have complications. One of these complications is hearing loss.Materials and Methods: Coronary artery bypasses grafting patients having inclusion criteria participated. One day before surgery, One day after surgery and if patients had hearing loss, the next week otoacoustic emission test was done. Patients were divided in to two groups: on PUMP and off PUMP bypass surgeries.Results: Hearing loss occurred in 12%. Hearing loss in on-PUMP patients was 14.3% and in off-PUMP was 6/7% (p=0.09). The relation of preoperative OAE test and final OAE was tested and it was significant (p<0.005). In male patients, hearing loss in on-PUMP operation was 18/6% (p=0.002) and in females in on-PUMP operation was 7/4% and in off-PUMP operation was 18.2 %( p=0.1). Hearing loss occurred in 4/2% in 1500, 2000 HZ; 12/5% in 2500, 3000 HZ and 83/3% in 3500, 4000 HZ.Conclusion: It after coronary artery bypass graft is rather Frequents (12%) and is more common in on-PUMP surgery, and occurs in high frequency. hearing loss is more common in male patients.

Abstract: 

Background: Despite improved outcomes after cardiac operations, delirium is a common neurologic complication after cardiac surgery. In this study, we aimed to investigate the association of postoperative delirium and mean arterial pressure (MAP) during CARDIOPULMONARY PUMP in patients after coronary artery bypass graft (CABG).Methods: 100 inpatients submitted to coronary artery bypass graft were evaluated prior to and after surgery. In order to investigate the association between mean arterial pressure and incidence of delirium, data were collected at pre intra, and postoperative periods. The patients were examined for delirium using Mini-Mental State Examination (MMSE).Findings: Preoperative delirium was detected in 34 patients (34%) and 74 patients (74%) had postoperative delirium. There was a significant positive correlation between total score of MMSE and mean arterial pressure during CARDIOPULMONARY PUMP (r=+0.22, P=0.02). Furthermore, there was a significant correlation between total score of MMSE and O2 saturation (r=+0.22, P=0.03).Conclusion: Present study suggests associations between incidence of postoperative delirium and mean arterial pressure and O2 saturation during CARDIOPULMONARY PUMP. Maintaining mean arterial pressure and O2 saturation of blood at physiologic levels during CARDIOPULMONARY PUMP may be associated with less early postoperative delirium and cognitive dysfunction.

Abstract: 

In many of heart patients coronary artery bypass surgery is the best choice for treatment. Two types of techniques are used in this type of surgery. One of them is on-pomp surgery and another is off-pomp surgery. Based on some studies, patients experience some complications after surgery, including: depression, anxiety and decreases in cognitive performance. In this study, the patients psychological conditions and memory functioning which is one of cognitive function are described before and after surgery in two groups. Descriptive-comparative method is used. Total of 62 patients participated in this study, convenience Sampling is used. At the first patients one day before and then two months after surgery were evaluated. Depression, anxiety and stress scale (DASS-21) and also, Wechsler Memory Scale (WMS-R) are used. Results showed that patients in both groups had similar preoperative psychological conditions. The groups showed differences in memory function, on-pomp group showed significant decreases in memory functioning after surgery (p<0/001). According to these results, it seems temporary heart failure and use of CARDIOPULMONARY PUMP device during surgery, leading to a decline in cognitive function. Because of these, should consider some interventions to improve memory function and psychological conditions of cardiac patients that undergoing cardiovascular surgery.

Abstract: 

Background: The study was performed to compare clinical outcomes of methylprednisolone and dexamethasone in prime solution during CARDIOPULMONARY PUMP after coronary artery bypass grafting (CABG). Materials and Methods: In this double-blind clinical trial, sixty-two patients who underwent CABG were divided into the control group (31 patients with dexamethasone in their prime solution) and the study group (31 patients using methylprednisolone in their prime solution). Duration of mechanical ventilation, length of intensive care unit (ICU) stays, bleeding rate (chest tube volume), arrhythmia, and need for inotropic intake were compared. Results: The mean duration of mechanical ventilation and ICU stay in the dexamethasone group were significantly higher than the methylprednisolone group and the mean bleeding rate in the dexamethasone group was significantly lower than the methylprednisolone group (p<0. 05). Conclusion: The mean duration of mechanical ventilation and ICU stay in the dexamethasone group was higher than the methylprednisolone group, but the use of dexamethasone has the advantage of being associated with less bleeding.

Abstract: 

Background and Objectives: Coronary artery bypass grafting (CABG) is usually performed through CARDIOPULMONARY bypass (CPB). CPB, However, has been associated with several adverse effects. Recently, off-PUMP CABG technique is offered as an alternative to the standard on-PUMP CABG technique. The purpose of this study was to evaluate the safety of the off-PUMP technique by estimating serum enzymes e.g. Creatin Kinase (CK), CK-MB, Lactate dehydrogenase (LDH), Aspartate Transaminase (AST), measurement of cardiac troponin I and some postoperative complications.Materials and Methods: Ninety patients (75 males and 15 females) with angiographically defined CAD admitted to Shahid Madani Heart Hospital in Tabriz for coronary artery bypass grafting were selected. The off-PUMP technique was performed in 50 patients with mean age of 58‏. In 40 other patients with mean age of 56, the on-PUMP technique was applied. The blood samples were collected from the patients before, 6, 12 and 24 hours after the operation. The activities of CK, CK-MB, LDH, AST in all serum samples were measured by Cobas Mira autoanalyser, and cardiac troponin I concentration also was measured by Evernest model 3200 motion. Data regarding patient age, sex, familial history of cardiovascular diseases, number of vessels grafted, procedural time, transfusion of blood and blood pressure were recorded for the both groups of patients. Postoperative data regarding the incidence of complications including myocardial infarction, need for inotrop adminstration and death were also recorded.Results: In this study the activities of serum enzymes such as CK, CK-MB, LDH and AST after on-PUMP CABG technique were higher than those of off-PUMP technique (P< 0.05 in all cases). The levels of troponin I were significantly higher after on-PUMP CABG throughout the entire measurement period (P<0.01) compared with off-PUMP procedure. In both groups of patients no myocardial infarction was occurred during operation. In the off-PUMP technique procedural time was shorter and transfusion of blood was less than those of on-PUMP technique, but no difference was noticed among the other recorded data.Conclusion: Significant reduction in the release of enzymes and troponin I during operation by off-PUMP technique suggests that the technique causes less myocardial injury. On the basis of little changes in the activities of cardiac enzymes and troponin I in serum and presented data including a marked decrease in blood transfusion and shorter procedural time, it is concluded that off-PUMP CABG is a safe and effective technique in selected patients with appropriate coronary lesions.

Abstract: 

Background: This study compared the effect of Voluven and Ringer solution in CARDIOPULMONARY bypass on the bleeding after the surgery and renal function in coronary artery bypass graft surgeries. Methods: Two types of prime solution including ringer-as Crystalloid-and Voluven were used in two groups and compared in terms of bleeding and renal function during the first 24 hours after the surgery in intensive care unit (ICU) cardiac surgery. Results: Amount of a 24-hour drainage was not statistically different (P > 0. 05). There was not any significant difference between groups regarding urinary output in two time periods, PUMP time and during the first 24 hours after the patient’ s entry to ICU. The mean of partial thromboplastin time postop in experiment and control groups was significantly different (P > 0. 05). Plasma level of Bun-Cr, sodium, Potassium, hemoglobin, hematocrit, BT, PT and INR Platelets was not significantly different during the first 24 hours after the surgery (P > 0. 05). There was a significant difference between the two groups in terms of a need for blood transfusion during the surgery and first 24 hours after the surgery (Ringer 1. 70 ± 0. 9, Vovuven 1. 07 ± 1. 1) (P = 0. 019). There was not a significant difference between two groups in terms of urinary protein secretion during the first 24 hours after the surgery (P = 0. 08). Conclusions: It can be concluded that using Voluven as prime solution in comparison to lactate, in our study did not cause any significant difference in bleeding after the surgery and there was not so much need for blood injection in these patients.

Abstract: 

Background: Vasoplegia or vasoplegic syndrome is a type of vasodilator shock that occurs in a large number of patients after cardiac surgery under a cardiac pulmonary PUMP, which can increase the incidence of complications and mortality in patients. Therefore, it is necessary to find a way to treat this disease. So, the aim of this study was to evaluate the effect of two doses of alpha- agonist midodrine on the prevention of vasoplegic syndrome induced by CARDIOPULMONARY bypass.Methods: This study was performed on 135 patients undergoing cardiac surgery (45 cases in midodrine 5 mg, 45 cases in midodrine 10 mg, and 45 cases in control group) in Chamran hospital, Isfahan, Iran, in 2016. Patients were randomly assigned into three groups, the drugs were prescribed one hour before surgery, and the vital sign and complications were recorded in patients. Chi- square and ANOVA tests were used to compare the data.Findings: The mean blood pressure during the PUMP was higher in the midodrine 10 mg group than the other groups (5 mg midodrine and control) (P=0.021); and the mean ephedrine administration in this group (15.33 mg) was also less than 5 mg midodrine (21.55 mg) and control (29.77 mg) groups (P=0.122).Conclusion: Considering the high incidence of vasoplegic syndrome after cardiac surgery, administrating 10 mg of midodrine can improve clinical symptoms during the PUMP and reduce the need for vasopressor drugs, which can make better surgical results.
